The instruction for getting to the trailhead past Index are incorrect. It is much farther up that road than shown in the instructions. After 20 minutes, we turned around and went the other way, which was 13 miles from highway 2, 4 on pavement and the rest on badly potholed gravel.
The hike was great! The new parking lot only had 5 cars when we arrived at 9:30. The view from the top was spectacular. It was hot on the exposed south facing open switchbacks near the top-a good hat and sunscreen are highly recommended.

The road from Index to the trailhead is (as it says on the page) paved for the first 13 miles, and then about 2 miles on gravel to Frog Mountain. I would not describe any part of it as badly potholed - I did this drive in a low clearance sedan with no problems.

This person likely went via NF-65/Beckler Rd and not via Index-Galena. Index-Galena Rd is nice.
